Design for a book cover with the back cover indicated on the left half of the page, and the front cover indicated on the right half. At the top center of the front cover is a large black number five resting on top of a horizontal dark blue bar. Below this [ON REVOLUTIONARY ART] is printed in black, capital letters. Beneath this is a bright red square, with [CONTRIBUTORS / 1 / 2 / 3 / 4 / 5] printed in black capitals inside it. About a centimeter off the right edge of the front cover is a vertical black line running the length of the page parallel to the edge. In the upper left corner of the back cover is large number five which is half white and half blue, with the upper white half inside a large mid-blue rectangle that stretches the length of the cover. Below this [ON REVOLUTIONARY ART] is printed in black, capital letters. At the bottom of the back cover a bright red, horizontal rectangle stretches the lengh of the cover. At the center of the design, two parallel graphite lines indicate the spine of the book.
